Primary Health Tasmania (Tasmania PHN) is a non-government, not-for-profit organisation working to connect care and keep Tasmanians well and out of hospital. We are one of 31 similar organisations under the Australian Government’s Primary Health Networks Program.
We engage at the community level to identify local health needs and work with health system partners and providers on innovative solutions to address service gaps, including through commissioning services.
We support general practice – as the cornerstone of the health care system – and other community-based service providers to deliver the best possible care for Tasmanians.
We are driving a collaborative approach to ensure people moving through all parts of the health system receive high-quality and streamlined care.
We are proud to work in partnership with the Alcohol, Tobacco and other Drugs Council Tasmania, Drug Education Network, consumer organisations, and other stakeholders towards better health and wellbeing for Tasmanians affected by alcohol and other drug use.
Carer Gateway, an Australian Government initiative, is the single contact point for carers providing unpaid care and support to a family member or friend with disability, mental ill health, chronic or life limiting illness, alcohol or other drug dependence or who are frail and aged.
Carer Gateway helps carers navigate the maze of supports, connect with other carers, get help at home, get support to take a break, speak confidentially to a counsellor or work through caring challenges with a coach. All services are free.
Care2Serve, the service arm of Carers Tasmania, is Tasmania’s Carer Gateway service provider. Care2Serve has offices in Burnie, Launceston and Moonah.
