Primary Health Tasmania (Tasmania PHN) is a non-government, not-for-profit organisation working to connect care and keep Tasmanians well and out of hospital. We are one of 31 similar organisations under the Australian Government’s Primary Health Networks Program.

Carer Gateway, an Australian Government initiative, is the single contact point for carers providing unpaid care and support to a family member or friend with disability, mental ill health, chronic or life limiting illness, alcohol or other drug dependence or who are frail and aged.

Carer Gateway helps carers navigate the maze of supports, connect with other carers, get help at home, get support to take a break, speak confidentially to a counsellor or work through caring challenges with a coach. All services are free.

Care2Serve, the service arm of Carers Tasmania, is Tasmania’s Carer Gateway service provider. Care2Serve has offices in Burnie, Launceston and Moonah.
